Office Policies




Patients are seen by appointment only.


Patients who do not reschedule or cancel appointments with 24 hours advanced notice or

do not show up for their scheduled appointment will be charged a $25.00 fee. The office

will make reasonable attempts to confirm appointments one day in advance of the

appointment date.


It remains the patient’s responsibility to keep or reschedule appointments in compliance

with the above policy. Exceptions will be made for medical or family emergencies. Please

note that insurance companies cannot be billed for missed sessions.





Payment (including copays) is due at the time of service. Outstanding balances will result in

interruption in nutrition counseling until all bills are paid. Payment options include the

 Credit card payment via PayPal prior to the visit


 Credit card payment via PayPal at the time of visit


 ∙ Insurance payment*



Returned checks are subject to a $25 fee.


Patients wishing to submit a receipt to insurance for direct reimbursement may request a

“superbill” (medical receipt) at the time of the visit.


*Karin Hosenfeld MS, RD, CSSD, LD will file insurance claims only if arrangements have

been made between Karin and the patient before the initial visit. Otherwise full payment is

expected at the time of the visit. Patients are responsible for any remaining balance not

paid by insurance. Please be advised that insurance does not cover copays as this is the

patient’s responsibility.





Patients whose insurance companies require a doctor referral for nutrition counseling

should contact the doctor’s office directly. Physician offices may fax referrals to Karin’s

private and secure fax at (972) 516-3794. All medical information will remain confidential.
